
High-Risk Countries
See the US list of high-risk countries here. If your dog has visited a high-risk country in the last 6 months, on top of steps 1 and 2, you will also need:
Scenario A
For dogs vaccinated inside the US
- •A Certification of U.S.-issued Rabies Vaccination form
- •Filled out by a U.S. Department of Agriculture (USDA)-accredited veterinarian
Please note: The Certification of U.S.-issued Rabies Vaccination form is NOT the same form as a regular rabies vaccination certificate you might get from your veterinarian to register your dog with your local jurisdiction. It is a specific CDC form.
Scenario B
For dogs vaccinated outside of the US
- •A Certification of Foreign Rabies Vaccination and Microchip form.
- •Filled out by your veterinarian and endorsed by an official government veterinarian.
- •A valid rabies serology Titer (blood) report from a CDC-approved laboratory.
- •A reservation at a CDC-registered animal care facility.
- –If you do not have a valid rabies serology Titer (blood), this must include a 28-day quarantine reservation.
- •An itinerary that has the dog arriving by air at the U.S. airport where the CDC-registered animal care facility is located.
- •A microchip that can be detected with a universal scanner, implanted before the dog gets its rabies vaccine. The microchip number must be included on all required forms and supporting documents.
- •Compliance with U.S. Department of Agriculture's (USDA) and your U.S. destination's regulations.
